Faith Family Clinic's vision is that all working uninsured people and their families have access to the medical care they need.
Our Goal
In order to address the healthcare disparities and lack of access to affordable medical care by businesses and individuals, we are offering a medical membership plan through our Patient Centered Medical Home as authorized in Section 1301 of the Affordable Care Act. This provision allows businesses to offer affordable healthcare for full and part time employees and for uninsured individuals to obtain quality medical care for a small monthly fee.
Our Commitment
MemberCare provides enrolled employees and their enrolled family members with access to the primary medical care they need, when they need it, at a price employers and/or their employees can afford. If you are not satisfied, you can cancel at any time with a thirty (30) day written notice.
Our MemberCare program is not just another card in employee's wallets! It is an investment that more than pays for itself with peace of mind, security, and affordable access to care. Faith Family Clinic offers unparalleled medical services that integrate physical, spiritual, emotional and social healing through a patient centered medical home.
MemberCare may assist employers in recruiting and retaining qualified employees. By providing employees with affordable access to quality, primary medical care, employers may also help reduce absenteeism and increase productivity.
Our Services
As a Patient Centered Medical Home, our medical providers are licensed Physicians, Nurse Practitioners or Physician's Assistants. While some are employed by Faith Family Clinic, others are volunteers who help us treat patients and keep our costs as low as possible. We provide comprehensive primary care, from treating acute health issues like the common cold and flu to broad concerns such as diet and exercise. Visits may include annual physicals and personalized coaching to create positive lifestyle changes such as weight loss, smoking cessation, stress management and family enrichment. Faith Family Clinic also offers patient education on a variety of health topics.
From diet and nutrition to age specific screenings, Faith Family Clinic providers will evaluate your current state-of-health, make recommendations and work with you to improve your future well-being. We also care for minor, urgent conditions, including simple fractures, sprains, cuts, acute illnesses, minor surgical procedures and other conditions that do not require hospitalization.
Faith Family Clinic providers incorporate national guidelines to provide patients with state-of-the-art primary care for diabetes, hypertension, hyperlipidemia, heart disease, asthma, arthritis, osteoporosis and many other chronic conditions.
Our no-cost services include:
- No office co-pays
- Lab work as needed
- In-Clinic procedures (stitches, wound care, simple castings) as needed
- Physical exams (including pap & cardio stress tests)
- X-Rays and EKGs, as needed
- Children's vaccines
- School/sports physicals for minors (under 18)
- Registration with Pharmacy Assistance Programs, if eligible
- Medication samples, as available
- Priority usage of $4 or $5 medications (Wal-Mart, HEB, etc.)
